the hole where the head goes on my long sleeve dress shirt is a little loose. Do you think that’s okay or it has to be tighter?

yes. this often gives off a little bit more of a casual look though as opposed to a tighter shirt which will look dressier.

it depends; if it´s just 1 cm. loose, that´s fine, but more than 1.5 cm. won´t look so good; try a smaller size
